Terms & Agreement to Engage Services

I, the undersigned, attest that I am the rightful owner of the above listed pet(s) and hereby authorize the veterinarians and staff at Cumming Veterinary Clinic (CVC) to provide treatment including examining, diagnosing and the prescription of medications in the best interest of my pet. I assume financial responsibility for any charges accrued while my pet is in the care of CVC and understand that payment of all fees are due at the time service is rendered. I understand that my pet(s) will be seen at CVC on an appointment basis and that my failure to keep scheduled appointments or cancel scheduled appointments with at least 24 hours' notice may result in a Failed Appointment fee up to the cost of a Wellness Exam or forfeiture of any fees previously paid to hold an appointment time. I understand that if I arrive 10 minutes late or more for a scheduled appointment, my appointment may be rescheduled or I may be asked to wait to be worked in as a “walk-in” visit.
